  • Primary Care Physician (PCP), MD / Plantation

    Job Description

    The Primary Care Physician (PCP) in our organization is expected to demonstrate:

    ‚ÄĘ The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient‚Äôs outcomes by building a trusting relationship and helping them change behaviors.
    ‚ÄĘ The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients.
    ‚ÄĘ The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams towards their performance goals.

    The PCP will be required to demonstrate the capacity to function independently and in collaboration with other external health care professionals and the Clinica Las Mercedes clinical and administrative staff. The PCP is expected to comply with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals.

    The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.

    ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

    • The PCP provides regular and preventative care for their patients.
    • The PCP is responsible for patient care and outcomes including proper communication with specialists and hospital attending providers, in addition to managing and monitoring their care team.
    • The PCP is responsible for the assessment, diagnosis, treatment, management, education, and the health promotion of their patients.

    Additional Job Description

    EDUCATION AND EXPERIENCE CRITERIA:

    • MD in Internal Medicine, Geriatrics, Family Medicine or similar specialty required.
    • Current and active MD licensure in the state of Florida is required
    • A minimum of 1-year clinical experience in geriatric, adult, or family practice setting.
    • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.
    • Must have a current DEA number for schedule II-V controlled substances
    • Verbal and written fluency in English and Spanish required.

    Position Summary

    Job Description

    he Driver is responsible for accurately and effectively delivering prescriptions and OTC products to customers in a timely fashion.  Drivers must also perform administrative tasks and represent the company in a professional manner.

    Responsibilities:

    1. Plans and organizes delivery routes by designated zone, to maximize efficiency and productivity for the day.
    2. Obtains the customer’s signature of the delivery receipt upon delivery of prescriptions or OTC packages.
    3. Responsible for collecting proper cost-sharing, if applicable, from customers upon delivery.
    4. Answers all delivery-related questions for customers, unless it is related to medication usage.
    5. Responsible for keeping the vehicle clean and neat.
    6. Operates vehicle in a safe and efficient manner and leaves an unattended vehicle in a secure location.
    7. Complies with all applicable traffic laws and regulations.
    8. Performs routine maintenance on delivery vehicles.
    9. Returns all undelivered packages to the pharmacy at the end of the business day.
    10. Responsible for not leaving any packages in the car unsecured.
    11. The vehicle must be locked at all times while the driver is making a physical package delivery.
    12. Ensures company tablets and cell phones are maintained in excellent condition and used for business purposes only.

    Job Description

    An X-ray technician takes radiographic images of specific parts of a patient’s body, which a physician has requested in order to make a diagnosis. They need a thorough understanding of imaging equipment and x-ray procedures and, as they are required to interact with patients, they should also have solid communication skills.

    Responsibilities:

    1. Taking x-ray images of the patients and developing the films to reveal their skeletal or bone structures for diagnosis
    2. Conserving compliance with the governmental protocols and ensuring the safety of patients and employees by using protective instruments and exercising safety measures.
    3. Controlling the x-ray video display and making sure that the picture quality is up to the mark by making appropriate contrast and density adjustments.
    4. Stabilizing the patients and ensuring preparedness by educating them about the entire procedure, appropriately positioning them on the examining table, and covering them with protective clothing.
    5. Representing the latest techniques, equipment, and procedures to educate the staff about them
    6. Studying the x-rays, computerized specimens, to create a detailed report of the findings and present them to the physicians
    7. Evaluating the dimension of the area to be radiographed.
    8. Making copies of the x-rays and keeping a precise record of the patients who have undergone the imaging procedure
    9. Coordinating the work schedules.
    10. Assist as a Medical Assistant as needed.
    11. Proficient in EMR

    Qualifications:

    • A two years associate degree or four years bachelor’s degree in any of the radiography programs.
    • Must have a current License/ Permit in X-Ray Technology from the State of Florida, Department of Health Services.
    • Knowledge of HIPPA medical privacy regulations and practices.
    • Strong attention to detail.
    • Previous medical office setting experience preferred.
    • Bilingual

    Job Description

    The Pharmacy Technician in the filling department is responsible for preparing and labeling prescriptions. In addition to the counting, weighing, measuring, and pouring of prescription medication or stock legend drugs and controlled substances including the filling of an automated medication system. Utilize pharmacy‚Äôs adjudication platform, Pharmaserv, to enter all required information in order to record all legal requirements for the preparation of prescription verification by Pharmacists. Be in compliance with regulations from the State of Florida Board of Pharmacy, local, state, and federal requirements as it pertains to prescriptions. Prescription Fulfillment Technician‚Äės responsibilities may also include communication with prescribers, customers, other staff, and any other entity required to complete prescription.

    Responsibilities:

    1. Assists the Pharmacist in reviewing, processing, preparing, filling, labeling, and dispensing of medications and supplies.
      2. Arranges work activities so that the pharmaceutical needs of customers are met.
      3. Responsible for compliance with Florida statute 64B16-27.420 Pharmacy Technician regulations from the State of Florida Board of Pharmacy.
      4. Answers phones, respond to questions and requests or refer them to appropriate staff for medication information.
      5. Verifies all data entry information in Pharmaserv.
      6. Maintains records by recording and filling physician’s orders and prescriptions.
      7. Whiles completing fulfillment functions, checks medications, and administration supplies inventory for out of date products removes products from inventory, and works with the appropriate recourse to record out of date products.
      8. Maintains a clean and safe pharmacy by complying with procedures, rules, and regulations.
      9. Communicates effectively with other professional and support staff in order to achieve positive customer outcomes.

      11. Assists customers, family members, and others with concern and empathy; respect their confidentiality and privacy, and communicate with them in a courteous and respectful manner.

    Qualifications:
